My personal information is wrong on my lab report, can it be corrected?

The information on your lab report is the information that was entered during the registration process. Once the sample results have populated online, no changes can be made to the information on the report as it creates a disparity between the information on the chain of custody for the samples received by the laboratory and the data reported. If your samples have not been analyzed by the laboratory and you are viewing the report heading prior to the laboratory receiving or analyzing your samples, please contact MMD’s offices for further instruction.
